Case of the Month: March 2025

Clinical history

A 55-year-old woman presented to the dermatologist with a four-month history of a distinctive pruritic, erythematous, vesicular rash on her extremities and perioral region. Over the past year, she experienced unintentional weight loss and intermittent gastrointestinal discomfort, and was recently diagnosed with type 2 diabetes mellitus. Laboratory tests revealed significant hyperglycemia. An abdominal CT scan showed a 3-cm, well-circumscribed mass in the pancreatic tail, with no evidence of metastatic spread.

Questions:

  • What is your diagnosis?

  • What are the cells of origin and the associated clinical syndromes?

  • What is the most common genetic syndrome associated with this group of pancreatic tumors?
